Deepening of the fold area is a concern that is cosmetic that is frequent. Called the parentheses by advertising or the cheek-lip grooves, they seem and deepen with age. They develop gets the cheek tissue over sags and aging, falling with the lip that is fixed. The folds evidence their degree of concern being among the two main areas in which fillers are used. In my practice, the folds and the lips are about even in terms of injecting able improvement that is asked. The treatment method for folds that are placing is to soften them by pushing them out. The inversion or v-shape of this fold makes any push from underneath significant and not enduring. In these folds, excision produces the best result most flattening but the scar is not a fantastic trade-off that is aesthetic. An approach to this inverted fold is release. Detaching the attachments that are dermal from the release is produced by thickness of the nasolabialfalte because of loosening the portion of the fold. This can be achieved with instruments but the entry incision for access causes a scar that is aesthetically acceptable. A cutting wire was developed for this application folds can have a ‘cable release’. Placed through two stabs the cable does a fantastic job of a release of the overlying fold. The results are impressive and it is a procedure which could be done under local anesthesia in the office.
Another fat that is effective approach is the fat or dermal-fat strip graft. By taking a strip of skin and fat from somewhere else in the body the outer layer of skin, epithelium, is removed first, the strip graft is thread to the nasolabial folds via a tiny nick incision at the top and bottom of the bend. This acts although it is not synthetic, like putting an implant. It is recognized that grafts have greater survival rates since the binder provides a conduit for capillary ingrowths than complimentary injected fat. Establishing a blood supply is for optimizing graft take the key. To use a strip fat graft, however, requires two things; an operating room experience and typically a harvest site. This is the way that I have seen for all these difficult folds that were inverted. Whether results bear out that it is far better than fillers remains to be seen. Nevertheless, the results are promising.
